After I noticed this, I was overwhelmed and worried because of the patient’s safety, whichbecause the morning nurse forgot to sign the medication chart. Firstly, as a third-year student, I am so anxious and intimidated to stepabout stepping up and communicatecommunicating to the morning staff, and askedasking her if she gave the medications that morning as it was not signed. Secondly, if I waswere to ask my buddy nurse she might think that onat my level, I should know what to do. So then I thought to myself I need to get the morning nursenurse's attention and askedask her nicely because I do not want to cause any harm to the patient, such as an overdose. In addition, for the nurse towould be made aware that it is poor documentation that can cause harm to patients. This indeed was a good decision and rectifies the issue that she. She clarified and signed the document saying that the medication was given.
