Dear Dr. Amanda,
Re: Mrs. Agnes Rosario D.O.B: 05.09.1972
Warm greetings! Thank you for seeing Mrs. Rosario, a-46-year olda 46-year-old nulliparous, married, school teacher;, whom I suspected issuspect of suffering from the polycystic ovarian disease.
She presented to me with a history of menorrhagia and also complained that she was having back and lower abdominal pain, which I could make out from her face that it wasfacial expressions were severe in nature .
Talking about her past medical history, she has been suffering from a range of signs and symptoms since 1998, such as irregular menstruation and acne for which she underwent treatment for a couple of months. BesidesIn addition, she also had menorrhagia in 1999, on and off.
Mrs. Rosario has quite a relevant family history; her. Her mother and maternal grandmother were patients ofboth diagnosed with polycystic ovarian disease, whilst her paternal grandmother was a diabetic.
She has no habit of taking any substance,; however, she takes painkillers quite frequently.
I would appreciate it if you take over the management of Mrs. Rosario onwardsonward.
Should any queries arise, please do not hesitate to contact me onvia phone or email.
Yours sincerely,
Dr. Hovrath
