I'm writing to inform you that for the last three weeks Sarah has been hospitalized at Memorial Hospital due to a Stroke whichstroke. This has now left her partially blind and barley able to see through one eye. and She has completely lost her ability to walk and is currently on Hospice and is now recieving in homereceiving in-home care through Optimal Hospice Care. With that said, upon checking her mail, I'm informing you we have recievedreceived your appointment letter and with my help had Sarah fillfilled out the documents attached. soSo, I'm kindly asking that you please contact Sarah to re schedule her annual decertification because as I can see, your notice that is dated December 16th but was not recievedreceived until just recently so. I'm sure you couldcan understand with everything she has been through and is going thruthrough another appointment is needed.
